Consent form settings
Editing each treatment’s form, how long a form lasts, and sending forms automatically before appointments.
Editing a treatment’s form
- 1Admin → Treatments, open the treatment and choose Edit consent form.
- 2Health questions: choose what a “yes” means: fine, caution, or stop (a stop needs a written clinical reason to go ahead). Add your own, pick from Add a standard question, or tap Use these questions for all services to copy them to every treatment straight away.
- 3Change the risks, aftercare and consent statement (Reset to standard form undoes your changes). Tap Use this form, then Save changes on the treatment. Nothing is saved until you do.
How long a form lasts
Admin → Treatments → Consent forms. Under When a new form is needed, set Repeat visits for the same treatment to Every visit, or Reuse for 1, 3, 6 or 12 months. Tap Save. Regular appointment series are covered by the first visit’s form for up to a year. Covered visits need you to confirm Nothing has changed, which is logged.
Sending forms automatically
For appointments booked at the desk or by phone, switch on Send forms automatically and choose When to send it (as soon as it’s booked, 1, 2 or 3 days before, or 1 week before), and whether to send a Reminder if it isn’t done. Then tap Save. Clients who book online fill in the form while booking. The link is for that appointment only and runs out at the end of its day. No email is sent if the form isn’t needed. Forms filled in through a link are encrypted in the client’s browser before they’re sent, so only people with the See clinical records permission can read the answers.

Good to know
- Changes to a form apply to new consents only. Signed ones never change.
- Use recommended settings sets sensible defaults in one go.
